Abstract

Objectives: To assess the efficacy and acceptability of colposcopy as a screening method and to compare the findings of VIA, HPV and colposcopy with histopathology for early detection of cervical lesions.

Methods: A prospective observational study conducted in a tertiary hospital, over 18 months. 100 symptomatic women, meeting the inclusion criteria were recruited randomly, and underwent clinical examination, VIA, HPV DNA and colposcopic evaluation, followed by histopathology and appropriate management.

Result: VIA had Sensitivity: 88.0, Specificity: 36.0, PPV: 31.4, NPV: 90.0 and Accuracy: 49.0 %. HPV DNA had Sensitivity: 76.0, Specificity: 50.7, PPV: 33.9, NPV: 86.4 and Accuracy: 57.0 %. Colposcopy had Sensitivity: 84.0, Specificity: 50.7, PPV 36.2, NPV: 90.5 and Accuracy: 59.0 %. Majority of patients with Invasive Squamous cell carcinoma were treated with radical hysterectomy whereas those with premalignant lesions with LEEP.

Conclusion: Colposcopy is an effective screening tool for early detection of cervical lesions and for providing appropriate treatment in the same sitting. Keywords: Colposcopy, Cervical carcinoma, premalignant lesions of cervix, Human Papillomavirus
